Florida's Young Scholars Student Application

You have reached the on-line location to submit your Florida's Young Scholars Application.  

In a few short steps, you will have completed your application.  To have your application submitted, you must complete all the steps.  Partially completed applications are not accepted.

As you fill out the application, you will be asked if you wish to include a picture. (This is not required.) To do this step on-line, you must use a Visa or Mastercard.  If you do not have a credit card, or would prefer to pay by check, you should use the paper copy of the application that you received in your envelope and send us a regular picture and check with your application.   

The information you choose to include on this application will appear in the Florida's Young Scholars 2010 edition book.  You will receive a Proof Sheet in the mail to make sure we have all of your information correct.  Please remember, we do not publish home street addresses or email addresses in the book. 

Parents will be asked to sign off on this application at two different points. 

Please Note: To submit an application, YOU MUST be nominated by your teacher.  If we have not received your name through our nomination process, you will not be included in our publication.  Only students who were nominated by their teacher and took home a letter from their teacher with their name on it are eligible to fill out this application.  While there may be other students who meet the qualifications we set to be named a Florida's Young Scholar, we can only accept those who were nominated through our process.

We realize it may be impossible to include all honors you receive or may receive at the end of the year. We suggest you choose those awards most important to you throughout your lifetime or simply select those you received during the current school year. It is your choice to decide how you wish your biography to read.

Additional Awards/Honors/Activities

In addition to the awards listed above, there may be awards you were not able to include or were more specific than what our list contained.  An example of this may be “1st Place, Baker County 4-H Fair” or “AGM Organ Age Champion – 10 Year olds.”  Another possibility to include in this section are items from the previous list that you could not include because you had already selected eight items.  You could write them in here.  We are looking for specific honors or awards in bullet like fashion.  You should not write this section in complete sentences.  We do edit this section to meet our standard formatting criteria.

For no charge, Florida's Young Scholars posts the names of the students who were named 2010 Florida's Young Scholars on its website.  This will be completed by September.  On our site, we include the child's name, grade, school and hometown.  We do not publish home addresses, email addresses or a child's picture on the Internet.  To be included on the site, a parent must give permission to have it included. 

Child's Picture

In our print edition, you may elect to include your child's picture with his biography.  In case you were wondering, in past years, roughly half of the participants choose this option.  To cover the extra space and processing, there is a $10 fee for this option.    To choose this option using our online application, you must pay by using either a MasterCard or Visa.  You may submit a digital picture for us to use or choose to mail a standard photograph.  Simply follow the prompts as directed to select the type of picture you would like to choose.
