ACTIVITIES
You will now have the opportunity to select honors, awards, and activities that you would like included in your biography. You may select 8 of the boxes below.
We realize it may be impossible to include all honors you receive or may receive at the end of the year. We suggest you choose those awards most important to you throughout your lifetime or simply select those you received during the current school year. It is your choice to decide how you wish your biography to read.

In addition to the awards listed above, there may be awards you were not able to include or were more specific than what our list contained. An example of this may be “1st Place, Baker County 4-H Fair” or “AGM Organ Age Champion – 10 Year olds.” Another possibility to include in this section are items from the previous list that you could not include because you had already selected eight items. You could write them in here. We are looking for specific honors or awards in bullet like fashion. You should not write this section in complete sentences. We do edit this section to meet our standard formatting criteria. You may include up to 100 characters that include additional awards or activities not already listed above.

PARENT or GUARDIAN SECTION
For this section, please make sure your parent assists you so that your biography can be properly processed. A parent or guardian must complete this section.
Before we can complete your child’s application, we have a few areas for parents to complete. We thank you for your time in completing this application.

For no charge, Lone Star State Scholars posts the names of the students who were named 2013 Lone Star State Scholars on its website. This will be completed by September. On our site, we include the child’s name, grade, school and hometown. We do not publish home addresses, email addresses or a child’s picture on the Internet. To be included on the site, a parent must give permission to have it included.

In our print edition, you may elect to include your child’s picture with his biography. In case you were wondering, in past years, roughly half of the participants choose this option. To cover the extra space and processing, there is a $10 fee for this option. To choose this option using our online application, you must pay by using either a MasterCard or Visa. You may submit a digital picture for us to use or choose to mail a standard photograph. Simply follow the prompts as directed to select the type of picture you would like to choose.

Pictures: Alternate Method
If you prefer, a hard copy picture can be sent to Lone Star State Scholars , PO Box 365, Birmingham, MI 48012. Photo must be marked on the back with the following information: Full Name, School, Grade, and State.

Note: JPEGs or TIFs only. Photo must be maximum 600 pixels on the longest side at 300dpi. File must not exceed 2 Megabytes.

Meets Program Requirements
To be a Lone Star State Scholar you must be:
A) Nominated by your teacher because you meet specific requirements this year, or
B) A Lone Star State Scholar from a previous year and you continue to uphold standards that brought you your original nomination.
If your child is a scholar from a previous year, you must certify that your child currently meets the following standards:
- Maintains a B average or higher;
- Is involved in community service, extra-curriculars, sports or other community activities;
- Exhibits good conduct.
As a parent, by digitally signing my name, I certify that my son or daughter meets the standards to be a 2013 Lone Star State Scholar. The information included in this application is accurate to the best of my knowledge. My child has my permission to be named a Lone Star State Scholar.
